Our aim is to offer holistic\, patient-centred care that addresses the physical\, emotional\, and social aspects of health. During your visit today\, you will get a firsthand experience of the following aspects: \nThe Patient Journey\n\nRegistration and Check-In: Understand the process a patient goes through when they first arrive at Merepark\, from registration to checking in for appointments.\nMeet the Clinical Teams: One Team – Partners\, GPs\, ANPs\, Nurses\, Additional Roles – how and who delivers Merepark clinical services.\nConsultation: Observe a simulated patient consultation to see how healthcare professionals communicate with and diagnose patients.\nFollow-Up and Care Plans: Learn about our approach to follow-up appointments\, referrals\, and the creation of personalized care plans.\n\nClinical Service Delivery\n\nConsultation Rooms: You will have the chance to visit our consultation rooms where our skilled healthcare professionals diagnose and treat a wide range of medical conditions.\nPreventive Care: Learn about the importance of preventive care\, including vaccinations\, health screenings\, and chronic disease management.\nPrescriptions and Medication Management: Explore the process of prescribing medications and how we ensure patient safety and adherence.\n\nNon-Clinical Environment Management\n\nMeet and Greet Desk/Care Navigation Hub: Get an overview of our Care Navigation Hub area where our ANPs and administrative staff manage appointments\, registrations\, and patient inquiries.\nMedical Records: Discover how we maintain and secure electronic health records to provide accurate and timely care.\nPractice Administration: Learn about the behind-the-scenes work that keeps our practice running smoothly\, including billing\, HR\, compliance and staff resilience.\n\nCommunity Engagement \n\nExplore our community outreach initiatives\, such as health education programmes and local partnerships that aim to improve the well-being of our patients.
